For all Transfusion Related Acute Lung Injury (TRALI) investigations there are 3 documents that must be completed as soon as possible once the patient is stabilized. There are specimen requirements as well. Please read the summary document titled “TRALI Investigation”  for the details of what is required. The  requisitions that need to be filled in and submitted can be found below.

1. TRALI Patient Data Form ( Print and send) 
2. Platelet Immunology Laboratory Form- ( Fillable form- print and send)
3. A completed Transfusion Reaction Investigation Form- CM105 ( Stocked on hospital unit)
